Two of the violations are being considered for escalated enforcement.
An agency spokesman told The Day of New London on Friday that one finding involved a "willful violation" regarding changes made to a safety report that left out information, making it "not complete and accurate."
Millstone spokesman Ken Holt called the apparent violations "historical in nature" and said they "do not reflect our current performance."
Gov. Dannel P. Malloy issued a statement calling the apparent violations "simply unacceptable" and urged Dominion to fully cooperate.
